OSHA investigates after man crushed by glass in Hamden

Officials from the Occupational Safety and Health Administration are investigating after an employee at Orchard Glass in Hamden was killed by falling panes of glass Friday evening.

Hamden police and fire officials were called to the facility on Edmund Street after getting word of a serious accident there around 8:45 p.m.

When emergency crews arrived, they said medical treatment was being given to 50-year-old Faustina Pena after 10 to 12 panes of glass weighing 3,000 pounds fell from a storage rack on top of him.

Pena was rushed to Yale-New Haven Hospital where he later died from his injuries. Police said Pena had been working in a storage area at the time with his brother.

OSHA was on hand Monday to assist with the investigation, and according to OSHA's website, Orchard Glass, also known as Insulpane of Connecticut, has been the subject of numerous violations.

OSHA inspected the company in 2007 and Orchard Glass, which manufactures everything from glass shower doors to windows in buildings, was more than $42,000 in fines.

However, it is unclear how much the company actually paid after a settlement was worked out.

Eyewitness News also found eight more violations after a 2010 inspection.

OSHA told Eyewitness News that the violations were addresses and many of the fines were reduced.

Officials said Orchard Glass will remain closed pending the results of the investigation. OSHA said the investigation should take six months to complete.
