CareInSync selected to provide technology to enable more efficient patient care, and to help reduce readmissions for six leading New Jersey hospitals
SANTA CLARA, Calif., Sept. 25, 2013 /PRNewswire/ -- CareInSync and the Visiting Nurse Association Health Group (VNA Health Group) are pleased to announce the selection of CareInSync's Carebook™ to manage patient care for the Central New Jersey Care Transitions Program (CNJCTP), New Jersey's first Community-based Care Transitions Program (CCTP). VNA Health Group, which recently celebrated its 100th anniversary, has a distinguished reputation for delivering the highest quality of care to patients in their homes across Central New Jersey. In government performance measures, VNA Health Group's members – Visiting Nurse Association of Central Jersey, Robert Wood Johnson Visiting Nurses, Cape Visiting Nurse Association and Visiting Nurse Association of Englewood – outperform other agencies in multiple categories at the state and national level. Cape Visiting Nurse Association and Robert Wood Johnson Visiting Nurses rank in the top 20 percent of the nation, and have been awarded Home Care Elite Status by the National Research Corporation, the largest patient satisfaction measurement firm in the United States. CareInSync's Carebook mobile care coordination platform will enable the VNA Health Group to extend its leadership position by connecting nurses, case managers and other aftercare professionals in real-time to deliver patient-centered collaborative care.
The Central Jersey Mobile Care Transition Network™ powered by CareInSync's Carebook mobile platform will be used to track and monitor high risk patients discharged from six prestigious hospitals, including nationally-ranked Robert Wood Johnson University Hospital, Robert Wood Johnson University Hospital at Rahway, CentraState Healthcare System, Raritan Bay Medical Center, Saint Peter's University Hospital, and Trinitas Regional Medical Center. Carebook™ is a HIPAA-compliant, mobile care coordination platform connecting care providers across settings in real-time to deliver patient-centered collaborative care. Using Carebook, care team members can securely communicate across the team to collaborate on risk assessments and evidence-based transition plans. As patients transition out of the hospitals, they will get "picked up" by the network. Transition Coaches will then utilize Carebook to coordinate referrals from one of eight participating community-based organizations (CBOs): VNA of Central Jersey, Robert Wood Johnson Visiting Nurses, Community Visiting Nurses Association, Holy Redeemer Home Care, Monmouth County Division on Aging, Disabilities and Veterans Services, Middlesex County Office of Aging and Disabled Services, Union County Division on Aging, and Jewish Family Service of Central New Jersey. Aftercare providers from these CBOs will then use Carebook on their mobile tablets to follow the patients and administer the Care Transitions Intervention® program. Finally, Carebook's analytics dashboard will provide real-time visibility into care process measures and the ability to track the progress of the high risk patient population post-discharge.
According to Kathleen McConnell, VNA Health Group's Vice President of Care Coordination, "Our goal is to improve the quality of care for over 4,000 patients, while reducing 30-day readmission rates across the six participating hospitals by at least 20 percent. Accomplishing this requires extensive communication and coordination between providers, coaching services that are efficiently targeted, and improved access in the post-acute care setting. CareInSync's Carebook product satisfies these requirements, while also providing the advanced analytical tools we need to assess and improve performance."
Steve Curd, CEO of CareInSync, adds, "The VNA Health Group clearly understands the importance of intelligent technology to the efficient delivery of care, and we are pleased to add this innovative team as a customer. We are proud to join with them in support of the CMS Partnership for Patients." Siva Subramanian, PhD, COO & founder of CareInSync, continues, "Solving healthcare's biggest problems requires new approaches, and unprecedented collaboration across disparate care settings. Carebook powers this collaboration through an evidence-based mobile platform, resulting in better care at lower cost. We look forward to working with the VNA Health Group and the six coalition partner hospitals to reduce readmissions, improve efficiency, and raise patient satisfaction across the state, through our proven Carebook platform."
